SUBJECT: Distribution of 1995 W-2
Your 1995 Form W-2 Wage and Tax Statement will be sent to your campus address rather than your home address this year. Your document will be sent to the check or Surepay mail code that is currently on record. You should receive your W-2 on or shortly after WEDNESDAY, JANUARY 24, 1996.
It is hoped that this change in our delivery process will result in a higher success rate on first time delivery of this important document and thus significantly reduce the number of employees who are inconvenienced because of unsuccessful home mail delivery.
If you do not receive a Form W-2 by WEDNESDAY, JANUARY 31, 1996, please contact Payroll Customer Service at 534-0665. A payroll representative will assist you in receiving your 1995 Form W-2.
