OFFICE OF THE ASSISTANT VICE CHANCELLOR - HUMAN RESOURCES February 23, 1999 KEY ADMINISTRATORS / KEY SUPPORT STAFF STAFF PERSONNEL POLICY MANUAL HOLDERS SUBJECT: UCSD 1998/99 Annual Service Awards Program and Ceremony I am pleased to announce that the UCSD 1998/99 Annual Service Awards Program and Ceremony will be held on Tuesday, May 18, 1999, at the Price Center Ballroom. This year's Ceremony will honor and recognize those staff employees who have achieved 10, 15, 20, 25, 30, 35, 40 and 45 years of service. Service Awards Program and Service Pin guidelines, including necessary forms, are accessible on the Human Resources Department's Policy Development and Quality of Work/Life web site: http://www-hr.ucsd.edu/~qwl/sac.html Information and forms accessible at this web site are as follows: *Service Credit Calculation Procedures for Service Pins and Vacation Leave Accrual Adjustments (Attachment A) *Service Credit Calculation Form (Attachment B) *Direct Mail Address for Other University of California Locations (Attachment C) *University of California and State of California Employment Service Verification Form (Attachment D) *Service Awards Ceremony Questions and Answers (Attachment E) *Service Credit Calculations Examples (Attachment F) Additionally, last year the Service Awards and Service Pin guidelines and forms were issued to all Policy Manual Holders. Employees eligible for recognition this year are those whose service achievement occurs between May 1, 1998 and April 30,1999. The Service Credit Calculation Form (Attachment B) has been developed to assist with the eligibility determination process. To ensure that all eligible employees are recognized at this year's Ceremony, the following information must be submitted to the Human Resources Department, Office of Policy Development and Quality of Work/Life, mail code 0922, or electronically to jcraig@ucsd.edu by Monday, April 19, 1999. 1. Mail Code Invitations to the Ceremony will be mailed to all eligible employees in early May 1999. If it is necessary to contact the Campus Payroll Office for an employee's pay record for service credit verification and computation purposes, please allow at least thirty (30) days for the Payroll Office to provide the requested information. Inquiries regarding the Service Awards Ceremony should be directed to Jonnie Craig-Winston, Director of the Human Resources Office of Policy Development and Quality of Work/Life at (619) 534-9659 or at jcraig@ucsd.edu. The presentation of service pins is not included in the Annual Service Awards Ceremony. However, to request service pins for eligible employees, please review the Service Credit Calculation Procedures for Service Pins and Vacation Leave Accrual Adjustments (Attachment A) and complete the Service Credit Calculation Form (Attachment B). Departments are encouraged to request service pins prior to submitting employees' names for the Annual Service Awards Ceremony.
