SUBJECT: The 2000-2001 UCSD Annual Service Awards Program and
This Notice is provided to announce the 2000-2001 UCSD Annual Service Awards Program and Ceremony. During this year's Ceremony the campus will honor and recognize those staff employees who have achieved 10, 15, 20, 25, 30, 35, and 40 years of service. The Ceremony will be held on Tuesday, May 22, 2001, 2:30 p.m. at the Price Center Ballroom.
Departments should submit the names of their staff employees with their
qualifying years of services by Friday, April 13, 2001. The Service
Awards Program and Ceremony Procedures and Service Pin Guidelines are
accessible on the Human Resources Department website at:

Accrual Adjustments (Attachment A)
Employees eligible for recognition this year are those whose service achievement occurs between May 1, 2000 and April 30, 2001. The Service Credit Calculation Form (Attachment B) has been developed to assist with the eligibility determination process.
To ensure that all eligible employees are recognized at the 2000-2001 Ceremony, departments must submit the following information to the Human Resources Department, Policy Development and Quality of Work/Life Division, mail code 0922, or electronically to email@example.com by Friday, April 13, 2001:
1. ELIGIBLE EMPLOYEE'S NAME
2. ELIGIBLE EMPLOYEE'S QUALIFYING YEARS OF SERVICE
3. ELIGIBLE EMPLOYEE'S SUPERVISOR'S
4. ELIGIBLE EMPLOYEE'S DEPARTMENT HEAD'S
5. ELIGIBLE EMPLOYEE'S VICE CHANCELLOR'S
If it is necessary to contact the Campus Payroll Office for an employee's pay record for service credit verification and computation purposes, please allow at least thirty (30) days for the Payroll Office to provide the requested information.
Invitations to the Ceremony will be mailed to all eligible employees in early May 2001.
The presentation of service pins is not included in the Annual Service Awards Ceremony. However, to request service pins for eligible employees, please review the Service Credit Calculation Procedures for Service Pins and Vacation Leave Accrual Adjustments (Attachment A), and complete and submit the Service Credit Calculation Form (Attachment B) to the Human Resources Department Records Units at mail code 0922. Although it is not a requirement for the Service Awards Program and Ceremony, departments are encouraged to request service pins prior to submitting employees' names for the Annual Service Awards Ceremony.
