SUBJECT:  Revisions to the UC Demographic Data Transmittal Form

The UC Demographic Data Transmittal Form has been revised to collect information on veterans now identified as WAR/CAMPAIGN/EXPEDITION. This group of veterans served on active duty during a war or in a campaign or expedition for which a campaign badge has been authorized. These veterans are now covered by nondiscrimination and affirmative action requirements.

The WAR/CAMPAIGN/EXPEDITION veterans category listed on the revised Demographic Data Transmittal Form has been implemented in a corresponding field in the Payroll/Personnel System (PPS), and appears on a related online screen.

Departments should order and begin using the revised Demographic Data Transmittal Form immediately, and discontinue using the now obsolete Affirmative Action Data Transmittal Form. The Demographic Data Transmittal Form may be ordered through the UC Human Resources and Benefits "fulfillment house," KP Corporation, and is listed on the University of California Human Resources Order form, which is accessible on the Human Resources website at http://www-hr.ucsd.edu/~qwl/forms
