SUBJECT: New Campus ID Badge for Employees
Effective immediately, UCSD will begin the issuance of a new employee ID card. Cards previously issued will remain valid and will not require replacement unless requested by the Department.
The card has been designed in a new format that can be worn as an Identification Badge. (A pocket clip and lanyard will be provided at the time of issue). The new design will become the standard offering for the campus and the only card available for new employees and affiliates. The cost of the card is $15.00 for both new and replacements. There will be no change to the Student ID Card at this time.
Request forms in PDF format are now available through Blink and paper forms are no longer available through the Storehouse. This is the first step in the transition to an on-line requisition process that is expected to be deployed by September 1, 2003. To download request forms and instructions go to:
Departments requiring staff to have the new badge design may have new IDs issued to current employees using the existing photo on their current ID Card. Simply forward a list of employee names and the index to be recharged, to the Campus Card coordinator at firstname.lastname@example.org. Once the cards are produced, Campus Card staff will notify the requesting office to coordinate card distribution.
