ALL ACADEMICS AND STAFF AT UCSD (including UCSD Healthcare)

SUBJECT:    UCSD Vehicle Accident/Incident Reporting

As you may know, every employee driving a University vehicle that is involved in an accident/incident must report it within 24 hours to Fleet Services, regardless of the extent of damage. This is done by completing the Vehicle Accident/Incident Reporting Packet found in the glove compartment of your UCSD vehicle. To capture pertinent information this is best done at the scene of the accident/incident.

If for some reason a Vehicle Accident/Incident Report packet is needed, you may obtain one from Fleet Services (x43485) or Risk Management (x44237). If you are unable to fill out a report at the scene, you may now complete an on-line form through Blink at: http://blink.ucsd.edu/go/accidentreport.

When completing a report, you will need to include your 4-digit vehicle number and the date the accident/incident occurred. If you need assistance with the Vehicle Accident/Incident Report process, you may contact Fleet Services. Fleet Services can also photograph the vehicle and assist you with damage estimates and repairs.

If University vehicle damages for an accident exceed the $500 deductible, please contact Risk Management at x44237 to submit a Vehicle Physical Damage claim as soon as possible.
