SUBJECT:    Service Awards - Extension of Name Submission Period

A notice was issued in June 2005 announcing this year's Annual Service Awards Program and requesting information on eligible employees. This follow-up notice is the final call to submit information and extends the submission deadline to Friday, August 26, 2005. I would like to thank all those who have submitted information to date as this allows us to begin our preparations.

Each year, UCSD celebrates the dedication of staff employees and their years of service with the UCSD Annual Service Awards. Employees are eligible for recognition when they have achieved 10 or more years of service, at five-year intervals (10, 15, 20, etc.).

The 2004-05 ceremony will be held Monday, October 31, 2005, in the Price Center Ballroom. Invitations to the awards ceremony will be mailed to eligible employees in early October 2005.

SUBMITTING AWARD RECIPIENT INFORMATION

To ensure all eligible employees are recognized, departments should complete and submit either the following Excel spreadsheet(preferred) or Word table, which detail all required information:

Please e-mail the completed spreadsheet or table to worklife@ucsd.edu by Friday, August 26, 2005.

ELIGIBILITY

Employees are eligible for recognition this year if their service achievement occurs between September 1, 2004, and August 31, 2005. To assist in determining eligibility, departments may use the Service Credit Calculation form used to request service pins. This information is available on the Service Pins Web page on Blink:

Due to the high volume of Service Credit Calculation forms received over this period, all service credit calculations may not be verified prior to the Service Awards due date. Therefore, departments should ensure lists of eligible employees are submitted by Friday, August 26, 2005, even if they have not received verification from the Human Resources' Records Unit.
