SUBJECT:    Revised Self-Identification Forms for New Hires

The Office of the President has issued two new forms for the on-boarding of all new hired academic and staff employees. The forms are required to comply with changes issued earlier this year by the U.S. Department of Labor Office of Federal Contract Compliance Programs (OFCCP) to the Vietnam Era Veterans' Readjustment Assistance Act (VEVRAA) and Section 503 of the Rehabilitation Act of 1993 as amended (Section 503).

The "Voluntary Self-Identification of Race, Ethnicity and Veteran Status" form (http://blink.ucsd.edu/_files/HR-tab/EOSAA/u5605-10_14.pdf) replaces the Demographic Data Transmittal form. As required by VEVRAA, all new hired employees are invited to self-identify their status as protected veterans. This form also consolidates the collection of race and ethnicity into one section.

In compliance with Section 503, the "Voluntary Self-Identification of Disability" form (http://blink.ucsd.edu/_files/HR-tab/EOSAA/voluntary-self-identification -of-disability.pdf) invites all new hired employees to self-identify their status as an individual with disabilities.

Updates to the Payroll Personnel System (PPS) and other related systems have been distributed to accept responses on the new forms. Effective November 1, 2014, the new forms must be used in the on-boarding of academic and staff employees. Additional information will follow regarding the transition to the use of the new forms.
