SUBJECT: | Physical Plant Services Electronic Work Request Process |
In order to offer our customers more flexibility in requesting work from
Physical Plant Services and to promote the campus initiative to reduce
paperwork, we are now offering an electronic Work Request process on the
World Wide Web.
There are two PPS electronic forms available at this time. One can be used
in place of the usual #FO 2212 multi-part form for requesting recharge
work, and the second, simplified form may be used to report any maintenance discrepancies in state-funded facilities. For your convenience you will
still be able to call the Service Referral Desk at extension 42930 to
request service.
These forms may be accessed by pointing your forms-capable browser at
either the Auxiliary and Plant Services home page at
http://apsadmin.ucsd.edu:1010/ or the Physical Plant Services home page at
http://apsadmin.ucsd.edu:1010/apsppshr.html and selecting the appropriate
hypertext links from there. Of course, we will still accept hard copy
requests mailed to us at mail code 0908, or faxed to extension 42386.
We have also implemented an electronic Customer Feedback Form on the PPS
page in order to give our valued customers a convenient, paperless method
to tell us how we are doing, so that we may continue to improve our level
of service. Please feel free to use it to share with us any concerns,
perceptions or expectations regarding our services, or call our Customer
Service Supervisor, Wendy Schiefer, at extension 40317, if you prefer.
We are committed to assuring your satisfaction.
Administrators and support staff are encouraged to share this information
with other employees in your areas who may wish to utilize these new
electronic processes.
