Distribution of 1996 W-2

Your 1996 Form W-2 Wage and Tax Statement will be sent to your campus
address to the check or Surepay mail code that is currently on
record. You should receive your W-2 on or shortly after FRIDAY,
JANUARY 24, 1997.

Last year this change in our delivery process resulted in a higher
success rate on first time delivery of this important document and
thus significantly reduced the number of employees who were
inconvenienced because of unsuccessful home mail delivery.

If you do not receive a Form W-2 by FRIDAY, JANUARY 31, 1997,
please contact Payroll Customer Service at 534-3240. A payroll
representative will assist you in receiving your 1996 Form W-2.
