SUBJECT:	Reminder of the New Workers' Compensation Procedures
The Workers' Compensation Program is governed by State law and it is
imperative that the following procedures are followed to avoid delays in
obtaining benefits and to avoid penalties and fines to the University.
EFFECTIVE January 1, 1998:
INJURED EMPLOYEE'S RESPONSIBILITIES: 1.	INITIAL notification of an injury MUST be made to the Workers'
Compensation Office no matter how minor the injury (534-4785 or 534-0136).
There is voice mail available on both phones to take messages at all hours
including weekends and holidays. 2.	Following notification to the Workers' Compensation office, advise your
supervisor of your work-related injury and keep the department apprised of
your status.
SUPERVISOR'S RESPONSIBILITIES:
1.	When an employee informs you of a work related injury, inquire if the
employee has reported the injury to the Workers' Compensation office. If
the employee is unable to report the injury to the Workers' Compensation
office, it is the responsibility of the supervisor to call the Workers'
Compensation office.
2.	Advise the employee to seek medical treatment.
3.	The supervisor must report any change in the injured employee's status
(absences, modified work, etc.) to the Workers' Compensation Office.
Supervisors are no longer responsible for providing or processing claim
forms. WORKERS' COMPENSATION OFFICE RESPONSIBILITIES:
1.	When required, the Workers' Compensation Office will provide a claim
form to the injured employee and will be responsible for completing the
employer's section of the form.
2.	The Workers' Compensation Office will provide a copy of the injury
