SHIP Waiver and Healthcare over Break

* Student Health Services will be open over break, but will observe the University holidays and be closed December 24th, December 25th, December 31st and Tuesday, January 1st. During the remainder of the break we will be open M-F 8am-4pm. Please call 858-534-8089, or your primary care provider group, for an appointment. * Don't miss the Health Fee Waiver deadline! The waiver period is currently open. The deadline is scheduled for Wednesday, December 19th, 2012. The deadline will be extended through Monday, January 7th, 2013, subject to a $50 late waiver fee. No waivers will be accepted after the late waiver deadline. * SHIP students: If you require medical care during winter break, you must have a written referral from Student Health Services prior to seeking any non-emergency or non-urgent care. Call the SHS Insurance Office to speak to an Insurance Coordinator for assistance.

* SHIP students: For emergency or urgent care services, please call Anthem Blue Cross customer service at 866-940-8306 to locate the nearest participating facility.
