The UC Office of the President has authorized a special "change" opportunity for faculty and staff who find they have a medical plan for 2014 that doesn't meet their, or their family members' needs. Please review the following instructions and resources to select a different plan. Campus Faculty and Staff who wish to select a different plan should submit a UPAY 850 Enrollment Form (http://atyourservice.ucop.edu/forms_pubs/forms_worksheets/upay850.pdf) to either the Human Resources Department Benefits Office (campus mail code 0926) or the Payroll Department (campus mail code 0952) no later than Monday, March 31, 2014 to enroll in a different medical plan. The effective date of the new plan will depend on whether the employee or enrolled family members have had medical and/or prescription drug services prior to submitting the UPAY 850 Enrollment Form. If the employee or a family member uses services, the rules and costs of the plan that the employee is in at the time of the service will be applied to the service. The chart below will assist with determining the effective date for the employee's new medical plan.
Please note that it will take 30 to 60 days for the insurance carrier to update its membership system and mail ID cards. Employees' January paychecks will have the deduction for the medical plan they defaulted to or selected during Open Enrollment. It will take 30 to 60 days for the medical plan deductions to be adjusted after the medical plan has been changed in the Payroll System.
