As an Equal Opportunity and Affirmative Action employer, UC San Diego is required to document disability and religious accommodations requested by University employees. An accommodation is any change in the work environment or in the way things are customarily performed that enables an individual with a disability and/or religious practice to experience equal employment opportunities. Examples of an accommodation include a modified work schedule, telecommuting, a leave of absence, providing equipment, and other relevant efforts.
There are a variety of accommodation efforts that are ongoing within
vice chancellor areas. In order to document these efforts, Campus Human
Resources has designed a tool that will assist with measuring UC San
Diego’s combined efforts. Effective July 15, 2015, when a department
receives a request for an accommodation by a University employee, the
completion of an Accommodation Services Log form is required. The
online form can be accessed at the following link: It should be noted that this form does not replace the current formal Interactive Process required for disability and religious accommodations requests. UC San Diego is committed to ensuring that the University maintains full compliance with its nondiscrimination and affirmative action obligations, and Federal regulations.
