SUBJECT: | New Electronic Health Record (EHR) Platform; Account Activation Now Available |
On August 19, 2019, UC San Diego's Student Health & Well-being will
complete a migration onto Epic-the country's leading electronic health
record (EHR) system currently in use by UC San Diego Health hospitals
and clinics.
This transition will provide patients of Student Health Services (SHS)
and Counseling and Psychological Services (CAPS) enriched connectivity
and access to the health care resources of the region's only academic
medical center, enhancing the outstanding clinical and mental health
services provided to you.
As a UC San Diego student, the benefits of this transition include:
* The MyStudentChart patient portal and mobile app, which allows you to
request appointments online, securely message providers, view test
results, request prescription refills, and in the coming months,
schedule online or tele-visits.
* Superior protection and security measures, safeguarding access to your
private and confidential medical and mental health information.
ACTIVATE YOUR ACCOUNT
To access the new system, please visit https://mystudentchart.ucsd.edu
and follow the instructions to activate a MyStudentChart account with
your UC San Diego AD/email login.
PRIVACY STANDARDS
It is of utmost importance that disclosures of health information by SHS
and CAPS providers are in accordance with student preference, Federal
and State laws, and ethical standards. Privacy questions may be directed
to the Office of Compliance and Privacy at 858-657-7487 or

Further inquiries regarding Student Health & Well-being's transition to
the Epic Electronic Health Record system may be directed to
