KEY ADMINISTRATORS/KEY SUPPORT STAFF

SUBJECT:    Recordkeeping of Voluntary Self-Identification of Disability Form

The U.S. Department of Labor's Office of Federal Contract Compliance (OFCCP) has issued additional information regarding compliance with Section 503 of the Rehabilitation Act of 1993 as amended (Section 503). Effective November 1, 2014, Section 503 requires that all new hires be invited to self-identify their status as an individual with disabilities (http://blink.ucsd.edu/go/identificationform). A campus notice regarding the new hire form is located at http://adminrecords.ucsd.edu/Notices/2014/2014-10-20-1.html

In order to comply with Section 503, all departments must also:

1. Maintain either a hard or electronic copy (PDF) of the completed form;

2. Keep the copy in a file separate from the employee's personnel file because it is confidential medical information; and

3. Keep the copy for 3 years.
